You'll be responsible for intensive case management services as identified by the ACT (Assertive Community Treatment) clinical team for individuals diagnosed with moderate to severe mental illness and co-occurring disorders.

Primary Responsibilities:
- Provide individual case management under clinical supervision, meeting productivity and quality requirements set by the Agency and within the limits of services permitted by position and scope of practice.
- Develop and maintain quality, accurate clinical charts for all clients served.
- Serve as a liaison between the hospital, criminal justice system, and other treatment services in the community.
- Adheres to standards pertaining to assessments and documentation.
- Work collaboratively with the multidisciplinary ACT team, including psychiatrists, nurses, social workers, and vocational specialists, to deliver comprehensive care.
- Establish and maintain relationships with community agencies, service providers, and other stakeholders.
