Chaplain

The Hospice Chaplain provides emotional, practical and/or spiritual guidance for patients and their families, meeting them where they are on their hospice journey. As part of the patient care team, chaplains support the unique needs of each patient, including visiting with patients and their families, coordinating with the clinical team and connecting them with community resources and services.

In the end-of-life planning have the privilege of is responsible for the provision of spiritual care services to patients and families/caregivers of St. Croix Hospice, either directly or through coordination of care with other spiritual counselors.

 

Essential Functions and Skills:

  • Conduct spiritual assessments and develop individualized care plans.
  • Provides direct spiritual support to patients and families.
  • Coordinates with community chaplains and spiritual counselors.
  • Maintains records for quality assurance and compliance.
  • Attends interdisciplinary team meetings and provides spiritual care consultation.
  • Recruits and educates community chaplains, counselors, and volunteers.
  • Assists in developing and maintaining a bereavement program, ensuring follow-up care.
  • Oversees condolence calls and support group scheduling.
  • Conducts funeral/memorial services as requested.
  • Organizes periodic memorial services for staff, volunteers, and clergy.
  • Supervises spiritual care volunteers as needed.
  • Participate in quality assessment and performance improvement activities.
  • Ensures HIPAA compliance and confidentiality of patient information.
  • Performs other duties as assigned.

 

Qualifications

Required:

  • Demonstrated theological coursework required.
  • If no degree, 5+ years of experience in a formal ministerial setting required.
  • Ability to pass background, consumer report and drug test.
  • Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

 

Preferred:

  • Bachelor’s degree in biblical studies, theology, or religious studies or equivalent theology degree preferred.
  • Graduate of accredited seminary or school of theology (MDIV, MTS, ThM or equivalent theological degree) preferred.
  • One or more unit(s) of Clinical Pastoral Education preferred.
