I.    DEPARTMENT

Day Service

II.    DESCRIPTION

A.    Purpose and Scope of Position

To provide individualized support to each person to work in the job of their choice, earn a wage in an integrated work environment.

B.    Responsibilities

    To participate as a member of team representing the individual’s work day.  Complete     vocational assessments and attend ELPs.  Carry out all individual programs and behavior     programs as written.  Administer medications following proper procedure.  Maintain all     attendance, payroll records.  Participate in on-going training.  Provide necessary     information to help complete time studies, if applicable.  Other duties pertaining to job as     assigned by supervisor, within the scope of the position.  Provide the individual support     for employment at work site. All work must be performed in a safe manner. 

    THERAP proficiencies, including but not limited to S-Comms; T-logs (new & view     daily);     GER’s (new, review); ISP data (enter); Health Tracking (new, search);     Attendance (enter).

Assist people to realize their personal goals by providing services and interventions which focus on the pursuit of personal goals and desires.

Ensure people are free from abuse and neglect by ensuring people are not subjected to actions, by anyone that causes them physical or emotional harm.

Ensure people choose where they work by supporting each individually through programmatic interventions which promote employment which matches their goals, desires, skills and aptitudes. 

Ensure people have any required modifications and adaptations which permit maximum use of their work environment.    

Ensure respectful treatment and interactions which enhance each person’s self-esteem and results in positive perceptions by others.

Ensure people work in environments that meet generally accepted expectations for safety, sanitation and emergency evacuation.

        Ensure no individual right is informally restricted. 

 

C.    Organizational Relationships

Supervised by site supervisor.  

D.    Site Specific

Each work site has unique work responsibilities related to the type of work at that site.  You will be expected to provide individualized support for each person assisting them to learn and participate in the work/job.  

Qualifications

III.    REQUIREMENTS

A.    Education and Training

High School education or equivalent.  Must be at least 18 years old. 

B.    Technical

Physically able to lift 50 lbs. or more.

C.    Experience

Working with people.
