Stavros has been serving the Persons with Disabilities Community for nearly 50 years carrying out its mission to help Consumers maintain their independence.
This community health nursing niche is integral to Persons with Disabilities (Consumers) who rely on our nurses to provide initial and annual assessments that allow for further services and resources to be identified by completing In-Home Evaluations for Consumers in the greater Western MA areas.
These evaluations bring skilled nurses directly to the consumer to complete an evaluation which helps to identify the needs and subsequent available services and resources which help consumers maintain their independence, including but not limited to PCM services, Housing Support, Skills Trainers who help to educate on the various services and resources.
LIST OF ESSENTIAL RESPONSIBILITIES
What you will do as a contract nurse at Stavros:
- Conduct initial and annual In-Home Evaluations
- Be familiar with Persons with Disabilities
- Have the liberty and flexibility to create your preferred case-load/schedule
- Complete assessments and case work and submit documentation in a timely manner
- Report and maintain communications with Clinical Director to track progress
