POSITION PURPOSE/SUMMARY: Responsible for the management and coordination of the Hospice House. Ensure continuity of care, cost effectiveness, and quality of operations within the facility. Facilitates communication and provides leadership to Hospice House staff and other areas within the agency. Directly supervise Hospice House Staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES/ACCOUNTABILITY
Responsible for coordinating and ensuring high-level, quality care for hospice patients requiring. Inpatient, Respite, and Routine level of care within the Hospice House, as appropriate for the needs of each patient. Collaborates with each member of the interdisciplinary team to fulfill the Hospice of San Joaquin Mission.

Ensures the Hospice House is compliant with all Policies and Procedures, and Hospice
Medicare Regulations and Joint Commission Requirements, and state and federal law.

Provides a safe environment for patients, guests, visitors, and employees.

Evaluates, identifies, and classifies tasks performed within the agency to
determine the exposure risk category.

Tracts and evaluates actual patient infections; records and maintains infection-specific data and reports to the QAPI Committee.

Makes recommendations for changes to practice and /or policies and procedures based on findings related to agency infection data.

Develops and implements a procedure to monitor potentially communicable infections reported by patient care staff members and ensures compliance with policies and procedures and current standards of practice related to exposure risk and the potential need for segregation from patients and other employees as applicable.

Takes action when actual exposures related to infectious diseases and or blood and body fluids occur, per agency policies and procedures.

Responsible for maintaining a Leadership role within Hospice of San Joaquin to fulfill the agency's mission. Consistently represents the Hospice House, both internally and externally, with integrity and professionalism.

Monitors documentation and implementation of physical, psychosocial, and spiritual
interventions for the patient/family/ caregivers.

Performs evaluations of patients for admission and/or transfer with SW for proper placement in the Hospice House at the correct level of care per CMS regulations.

Responsible for interviewing and hiring qualified Hospice House staff in accordance with regulatory and agency requirements. With the nurse.

Ensures that all shifts are covered by appropriate disciplines to meet the needs of patients.

Manages Paycom for staff timely and effective with communication from the Charge Nurse.

Back up to the charge nurse for staff coverage for PTO and Sick calls. Reports need to cover any shift with outside agency staff to the CCO.

Collaborate with other Clinical Supervisors and/or the CCO as needed if issues arise related to staff involved in the care of Hospice House patients, but not directly supervised by the Hospice House Manager.

Oversees charge nurse. Completion of employee evaluations on time.

Oversight of the charge nurse, Responsible for Employee Performance Improvement activities per agency policies and procedures.

Responsible for the overall management of the Hospice House, including but not limited to
 budget preparation, expense approvals, contracting issues, maintenance of supplies, and other management responsibilities as appropriate and designated by the CCO or CEO.

When the charge nurse is not available, HHM is available to the Hospice House staff 24/7 for assistance with immediate patient care needs and or reporting requirements related to incidents or adverse events in accordance with agency policies and procedures.
