What You’ll Do
- Conduct initial and ongoing nursing assessments for hospice patients
- Manage pain and symptoms to improve quality of life
- Educate and support families and caregivers throughout the care process
- Collaborate with the interdisciplinary hospice team and primary care physicians
- Maintain timely and accurate clinical documentation
- Travel to patient homes across your designated area within Macomb County
