Key Responsibilities

Provide psychosocial support, counseling, and resource coordination for hospice patients and their families.

Assess emotional, social, and environmental factors impacting care and develop individualized care plans.

Collaborate closely with the interdisciplinary hospice team to address patient needs and support holistic care.

Assist patients and families in navigating end-of-life planning, community services, and grief resources.
