Position Summary

The Hospice Medical Social Worker plays a vital role in supporting patients and families through emotional, social, and practical challenges during hospice care. As a full-time member of our interdisciplinary team, you will provide counseling, coordinate community resources, and advocate for patient and family needs—ensuring each individual experiences compassionate end-of-life care that honors their values and choices.

Key Responsibilities

  • Provide psychosocial support, counseling, and resource coordination for hospice patients and their families.

  • Assess emotional, social, and environmental factors impacting care and develop individualized care plans.

  • Collaborate closely with the interdisciplinary hospice team to address patient needs and support holistic care.

  • Assist patients and families in navigating end-of-life planning, community services, and grief resources.

  • Advocate for patient rights and ensure care aligns with cultural, spiritual, and personal preferences.

  • Maintain accurate, timely documentation in compliance with hospice regulations and agency standards.

Qualifications

Qualifications

  • Education: Master’s degree in Social Work (MSW) required.

  • Licensure: Must hold a Michigan Limited or Full Master Social Work License (LMSW/LLMSW).

  • Experience: Minimum 5 years of hospice or palliative care experience required.

  • Excellent communication, counseling, and interpersonal skills.

  • Strong understanding of Medicare hospice regulations and psychosocial best practices.

  • Compassionate, dependable, and emotionally resilient professional approach.

  • Proficiency in electronic medical records (EMRs); experience with Axxess or Home Health Notify preferred.

  • Valid driver’s license and reliable transportation.
