Job Summary

We are seeking a skilled and compassionate Occupational Therapist (OT) to join our Home Health team on a part-time basis. This position is ideal for clinicians who are passionate about helping patients regain independence and improve their quality of life. You’ll visit patients in their homes across Oakland and Wayne Counties, performing evaluations and follow-up visits while collaborating with a supportive interdisciplinary care team.

Responsibilities

• Conduct patient assessments and develop individualized care plans

• Implement therapeutic interventions to improve mobility, daily living skills, and functional independence

• Educate patients and caregivers on exercises, home safety, and adaptive equipment use

• Coordinate with physicians, nurses, and other therapy disciplines to ensure comprehensive care

• Accurately document all patient visits and progress in the EMR system

• Provide compassionate, patient-centered care aligned with home health standards

Qualifications

• Active Michigan Occupational Therapist (OT) license

• Minimum 1 year of experience in Home Health or related setting preferred

• Strong interpersonal, clinical, and communication skills

• Reliable transportation and a valid driver’s license

• Familiarity with EMR systems (experience with Axxess or Home Health Notify preferred)

Background Screening

Safe Hands Home Health Care & Hospice conducts background screenings on all candidates through the Michigan LARA (Licensing and Regulatory Affairs) system upon acceptance of a contingent job offer.
