Job Summary
We are seeking an experienced and compassionate Hospice Registered Nurse Case Manager (RNCM) to join our growing hospice team. This role is ideal for nurses who are passionate about providing holistic, end-of-life care while managing the clinical, emotional, and educational needs of patients and their families.
As an RN Case Manager, you will oversee and coordinate care for hospice patients, ensuring their comfort, dignity, and quality of life. You will serve as a trusted point of contact for patients, families, and interdisciplinary team members—making a meaningful difference every day.

Responsibilities

Provide comprehensive nursing assessments and individualized hospice care plans

Manage all aspects of case coordination, ensuring continuity and quality of care

Monitor patient symptoms and adjust care interventions as needed

Offer education and emotional support to patients, caregivers, and families

Collaborate with physicians, social workers, chaplains, and other team members

Document all assessments, interventions, and communications accurately in EMR systems

Participate in interdisciplinary team meetings and quality improvement activities

Maintain compliance with hospice regulations and agency policies
