ID#6670A - The Peer Advocate participates as a part of the PIER Treatment Team to provide peer support and linkage to community resources. One of the two Peer Advocates in the PIER Program will also be designated to train on the UC Davis data collection tablet application specific to the PIER Program and will coordinate all internal PIER Program data collection to ensure all clients complete outcome measures electronically.
DUTIES AND RESPONSIBILITIES:
Provide Peer Support and Case Management Services with clients and family members with a focus on recovery and wellness. Will screening for case management needs and linkage to services within the Center and within the community.
Co-facilitate Multi-Family Groups with PIER Mental Health Clinicians in multiple locations within the community. Participate in Community Outreach Services (COS) to engage and provide information to individuals and organizations in the community about linkage to the PIER Program serving individuals exhibiting early high-risk signs of psychosis.
Complete and submit all required DMH and Center paperwork within the required timelines, submitted in a complete and thorough manner. One of the PIER Program Peer Advocates will be responsible for training in the outcome measures tablet application and ensuring outcome measures are completed for all clients, and all data is entered in the tablet and submitted as per training requirements with UC Davis.
Provide support and linkage with parents/caregivers to assist with accessing community resources and referrals, and assist parents/caregivers in understanding the nature of the services provided and their roles, rights, and responsibilities. Peer Advocates will be available to accompany clients/families to activities to model problem solving and self-advocacy skills that empower and foster independence.
Consult with youth on activities of interest and scheduling needs to address potential barriers such as transportation and childcare.
