Description

ID#36140A - Provide supportive counseling and case management services within the scope of practice, utilizing both individual and group modalities. Deliver crisis intervention services to address immediate concerns and risks, offer initial support and assistance, and foster a therapeutic relationship between the client and the clinic.
Key Responsibilities
  • Maintain a caseload of at least 50 clients.
  • Provide Officer of the Day coverage for crisis and walk-in clients as scheduled.
  • Screen clients for intake appointments and/or referrals.
  • Participate as an active member of a multidisciplinary treatment team.
  • Attend all scheduled clinical debriefings, staff meetings, and team meetings to assess client needs, strengths, and resources, and contribute to treatment planning.
  • Complete all required documentation—including charts, reports, service plans, needs assessments, and correspondence—within agency and Department of Mental Health/Medicare guidelines and timelines.
  • Collaborate professionally and effectively with clinical and psychiatric staff.
  • Network with community resources to facilitate client linkage and referrals.
  • Conduct home visits when necessary and safe.
  • Work at least one evening per week.
  • Perform other duties as assigned by the Program Manager.

Qualifications

Minimum Qualifications
  • Bachelor’s degree in psychology, social work, or a related field; master’s degree preferred.
  • Minimum of two (2) years of professional experience working with chronically mentally ill individuals and providing crisis intervention.
Additional Requirements
  • Must comply with all HIPAA regulations and related state confidentiality laws.
  • Successful completion of the Center’s HIPAA training, including passing the HIPAA test and receiving Certification of Compliance.
  • Regular attendance, punctuality, and active engagement with colleagues are essential functions of the role.
  • Demonstrated knowledge of:
    • Chronically mentally ill populations
    • Crisis intervention techniques
    • Community resources
    • Social Security and General Relief (GR) benefit programs
  • May be required to hold a valid California Driver’s License and be insurable under the Center’s auto insurance policy.
  • Spanish language proficiency is desirable for certain positions within this classification
