WOODLAND POND CONFORMANCE STATEMENT

POSITION SUMMARY
The Woodland Pond Geriatric Licensed Clinical Social Worker (“GLCSW”) will be directly supervised by the President and CEO. The primary role of the GLCSW is to work with the Woodland Pond interdisciplinary team, residents, and their loved ones to improve the quality of life of the Woodland Pond residents and help alleviate some of the negative aspects of aging. To help the residents enjoy happier, more productive lives.
ESSENTIAL DUTIES AND RESPONSIBILITIES (*CF indicates Critical Function)
- Assesses the psychosocial needs of the Woodland Pond residents and identify opportunities for interventions when interventions are needed*
- Recognizes the differences between normal and abnormal aging patterns
- Helps residents deal with their problems and alleviate some of the pain of aging, including by providing one on one our group counseling or support services*
- Provides inter-relationship support between significant others, family members, loved ones, neighbors, etc when challenges arise*
- Provides support with the many types of transitions that residents face during their lives at Woodland Pond
- Supports the admission and discharge planning processes at Woodland Pond
- Maintains Clinical Social Work license in NYS in good current standing at all times

- Completes all required training and in-services
- All other duties as assigned

DISRUPTION TO ROUTINE
Must be able to adapt to resident and staff member interests and occasionally conflicting needs. Occasional schedule changes due to resident and staff requests and/or special events. Occasional additional hours. Occasional evening and weekend hours. Frequent interruptions
SAFETY HAZARDS IN THE JOB
Stress due to need to meet multiple, varying needs of residents and staff and need to meet deadlines. Eye and position strain from work on computers
