POSITION SUMMARY: Â Provides social work services to patients and families in both the HomeCare and Hospice programs. The Social Worker assesses and addresses social, emotional, and practical needs related to illness and care. Participates as an integral member of the interdisciplinary team, assisting with treatment planning, discharge planning, community resource coordination, and completion of psychosocial assessments, screenings, and advanced care planning.
Masters-level social workers may provide supervision and oversight for bachelor-level social work staff, in accordance with regulatory and organizational requirements.

DUTIES & RESPONSIBILITIES:
 Patient Care & Assessment
- Responds to referrals for social work services within Schoolcraft Memorial HomeCare & Hospice.
- Conducts comprehensive psychosocial assessments through chart review, patient/family interviews, and collaboration with the interdisciplinary team.
- Develops individualized psychosocial treatment plans based on patient/family needs, regulatory requirements, and reimbursement guidelines.
- Provides counseling, education, and emotional support to patients and families to address adjustment, coping, and end-of-life concerns.
- Maintains regular visit frequency as outlined in the patient’s plan of care.
Interdisciplinary Collaboration
- Participates in interdisciplinary team meetings and case conferences to discuss psychosocial issues affecting patient progress and care outcomes.
- Communicates with attending physicians and care team members regarding psychosocial progress and changes in patient status, obtaining treatment orders as needed.
- Collaborates with internal departments, medical staff, and community agencies to coordinate services and resources supporting the plan of care.
- Provides consultation and guidance to clinical staff on communication, conflict resolution, and psychosocial dynamics affecting patients and families.
Documentation & Compliance
- Accurately documents assessments, interventions, and patient/family responses in the medical record per agency documentation standards.
- Completes all required reports, forms, and records promptly and accurately.
- Adheres to Schoolcraft Memorial HomeCare & Hospice policies, procedures, and quality improvement standards.
- Maintains confidentiality and complies with HIPAA and organizational privacy requirements.
Program Coordination
- Coordinates and implements the Hospice Bereavement Program, ensuring ongoing support to families following patient death.
- Oversees and schedules Hospice Volunteer activities in alignment with program needs and regulatory guidelines.
- Provides supervision and oversight to social work assistants and bachelor-level social work staff, ensuring quality and compliance.
Professional Development & Organizational Contribution
- Participates in agency meetings, in-services, and continuing education to maintain professional competence and regulatory compliance.
- Maintains current knowledge of community resources and fosters strong working relationships with partner organizations.
- Contributes to the agency’s Quality Improvement Program and overall operational goals.
- Demonstrates flexibility and adaptability to changes in policy, procedure, and scheduling.
