The Care Manager provides service coordination for children who have been diagnosed with serious emotional issues, developmental disabilities, chronic medical conditions, or have experienced complex trauma.
Responsibilities:
- The Care Manager serves as a single point of contact to manage, on a continuous basis, all of the unique medical, behavioral, and social needs of families.
- The program supports children in the least-restrictive home or community setting while linking children and their families to services which provide opportunities for improving health and well-being.
- They assess the need for services, develop service plans, complete home visits and link the child and caregiver to identified services.
- The Care Manager assists in stabilizing the child's environment and advocates for the child and family’s needs.
- Care Manager’s responsibilities are established by the Department of Health and the Children’s Health Home of Upstate New York.
