What We Need From You

Barnes Healthcare Services is seeking a Clinical Transitional Liaison (CTL) that will be assigned to one or more facilities in the Atlanta sales territory. The CTL supports the sales efforts of the Territory Manager (TM) by working in collaboration with case management and social work staff at the assigned facility. The expectation for this position is that the CTL will travel daily to the facility or facilities to develop relationships with discharge planners and case managers and review the needs of the patients being referred to the home setting or requiring ambulatory infusion care. The CTL provides continuous evaluation of the effectiveness of treatment through ongoing assessment of the patient. The CTL provides the therapy to teach, train, and instruct in the hospital prior to discharge to the home setting. The CTL communicates timely with the discharge planner and/or social worker to coordinate the needed care for patients at home.  Effectively communicates with sales and operations teams regarding patient needs and any issues with the facility to which they are assigned.


  • Under the supervision of the Regional Sales Manager, builds and expands relationships with new referral sources.
  • Maintains and expands relationships with existing referral sources.
  • Broadens the number of referral sources within a given hospital or other referral location, such as doctors, social workers, discharge planners, etc.
  • Obtains appropriate referrals for Barnes Healthcare Services.
  • Provides Teach, Train, and instruction to the patient pre-discharge to assure a seamless transition from hospital to home.
  • Works directly with families and hospital staff to gather appropriate information for referral.
  • Establishes and maintains relationships with Barnes's internal operations team and the referral sources to achieve efficient referral processing.
  • Performs appropriate clinical assessments as required.
  • Problem solves for referral sources.
  • Provide nursing assessment and use CPR+ system for clinical documentation.
  • Maintains a thorough understanding of confidentiality and HIPAA guidelines.
  • Understand the impact of cultural competency in working with diverse families and facility staff.
  • Attends facility functions to maintain a positive perception of Barnes Healthcare Services.
  • Serve in an administrative nursing capacity in compliance with regulatory guidelines and state and federal agencies, as needed.
  • Other duties as assigned


  • Must have a current ARNP/RN/RT/LPN license.  An additional out-of-state license(s) preferred.
  • Must be trained in all of Barnes Therapies and be able to instruct the patient and caregiver pre-discharge.
  • Must have exceptional communication skills.
  • Must be entrepreneurial in spirit and self-motivated.
  • Proven team player and able to work in a fast-paced environment.
  • Able to work independently.
  • Ability to perform successfully in a highly visible position.
