Rainbow Community Care is currently accepting applications for a dedicated Resource RN. This is a full time, hourly position that will be based out of the Inpatient Center in Johnson Creek.


Essential Duties include, but may not be limited to:

  • Prioritizes calls and instructs nursing staff to see patients as needed.
  • Provides direction to patients, families, and caregivers based on the plan of care.
  • Evaluates the needs of patients and caregivers to determine if it is appropriate to send clinical or social services staff to assess and treat.      
  • Ensures timely documentation of all calls, including problems and interventions.
  • Assists with scheduling tasks for RNs, SWs and CNAs, as directed.
  • Works closely with admissions team to ensure timely, efficient, and effective admissions.
  • Performs intake duties in collaboration with intake staff to cover vacations or during periods of high referrals.
  • Coordinates admissions to palliative care and hospice programs in collaboration with corresponding teams.
  • Performs referral, admission, palliative care, and other patient visits as needed to support the departments.
  • Monitors and manages patient equipment and supply needs.
  • Possesses knowledge of Medicare Conditions of Participation and HFS 131.
  • Maintains communication with patient, family/caregiver, attending MD, and others as necessary.
  • May be asked to deliver nursing and/or CNA care to patients including personal hygiene, comfort measures, nourishment, and provides physical and psychological support to the patient and family when staffing dictates.
  • Teaches patients, family members, and caregivers regarding the disease process, self-care techniques, end of life care, and process for dealing with grief and ethical issues.
  • Other duties and responsibilities as assigned.

Professional & Personal Qualifications

  • Wisconsin RN license is required.
  • Valid WI driver’s license and current auto insurance is required and must be maintained.
  • Hospice, home health, ER, Oncology, and/or palliative care experience is preferred.
  • Must present self in a professional manner at all times.
  • Excellent work ethics required, including good attendance and punctuality.
  • Strong written and verbal communication skills required.
  • Ability to maintain patient, family, and employee confidentiality.
  • Ability to apply problem-solving techniques in resolving issues and assist staff with decision-making and clinical direction.
  • Ability to drive to and from patients’ homes and to meetings at other locations as necessary.
  • Ability to maintain healthy boundaries and maintain positive interpersonal relationships.
  • Ability to be self-directed and handle stressful and emergency situations.
