ROLE SUMMARY:
The Home Visitation Program Supervisor will provide ongoing, intensive, professional supervision to all Home Visitors focusing on supporting healthy attachments and utilizing the Healthy Families America reflective strategies.

 

ESSENTIAL FUNCTIONS:
• Provides a minimum of 1.5 to 2 hours of individual supervision per week for each direct service staff member as required by the Healthy Families America national office.
• Shadows direct service staff to monitor and assess performance, provides constructive feedback and supports skill development.
• Conducts field supervision with Home Visitors and owns reliable transportation for travel to family homes and other community meetings.
• Is responsible for monitoring all aspects of Targeted Case Management provided by the supervisee.
• Tracks and assigns referrals as well as engages new referrals.
• Engages in various outreach activities which may include participating in community events and maintaining partnerships with community agencies for direct referrals.
• Continually monitors the effectiveness and efficiency of home visitation completion rate efforts. This includes client case load assignments and schedule reviews so that each client is actively being served, engaged, and re-engagement of clients.
• Assures that the supervisee is cognizant of and attending to issues of safety, including but not limited to, domestic violence, child abuse and neglect, elder abuse and neglect, cutting, running away, unsafe sex and the related impact of substance use/abuse.
• Is available for crisis intervention, including, but not limited to dangerousness assessments, Child and Elder Abuse, Tarasoff situations, psychiatric hospitalizations, domestic violence, runaway situations, substance abuse, cutting, running away, unsafe sex and other police involvement.
• Works collegially and cooperatively with the Quality Management and Evaluation Division in assuring supervisee documentation meets Wellnest and Home Visitation Standards.
• Maintains a working knowledge and compliance with all rules, regulations and supervision requirements according to the Healthy Families America model.
• Attends meetings with clients/families as appropriate (e.g. handling complaints, safety issues) after consulting with the Assistant Director and Division Director.
• Performs other tasks as assigned.

 

REQUIRED QUALIFICATIONS:
A minimum of a Bachelor’s degree (B.A. or B.S in child development or related field); Masters Degree is preferred. A minimum of 3 years of supervisory experience preferred. Bilingual Spanish speaking preferred.
Demonstrated passion for and commitment to assist individuals accessing services that lead to increased wellness. Knowledge of and ability to quickly learn multiple LA County systems is also required.

 

CONDITIONS OF EMPLOYMENT:
Employee may be asked to pursue additional training when it is determined to be in the best interest of the Company. Employee must have the ability to use a personal vehicle with the required insurance coverage to complete the essential duties of their job and conduct agency business. Employee must maintain a valid California driver’s license and clean driving record that meets Wellnest’s insurance carrier’s criteria for liability coverage. In keeping with Company policy for all employees, the successful applicant must also complete a test for TB, and fingerprint clearance by the Department of Justice. Proof that you will be fully vaccinated against COVID-19 and boosted per eligibility guidelines by your employment start date. Per the CDC and Department of Public Health, individuals are considered fully vaccinated for COVID-19: two weeks or more after they have received the second dose in a 2-dose series (Pfizer-BioNTech or Moderna or vaccine authorized by the World Health Organization), or two weeks or more after they have received a single-dose vaccine (Johnson and Johnson [J&J]/Janssen). The Company will provide exemptions in accordance with applicable law.

 

WORKING CONDITIONS:
Employee regularly works out in the field and in an office environment. Employee may be required to work flexible, evening and/or weekend hours for time-sensitive projects.

• Ability to use computer keyboard up to 75% of the day.
• Visual acuity, ability to adjust focus and peripheral vision for work on a laptop or other computer equipment.
• Lift and/or move up to 50 pounds.
• Able to use telephone, office copier, calculator, fax machine, computer printer and scanner, or other equipment as required.

COMPENSATION AND BENEFITS

  • Annualized Salary Range: $62,100 to $72,433
