JOB SUMMARY
Reporting to the Health & Wellness Director, the Licensed Practical Nurse (LPN) ensures the general well-being of residents by providing a system of services for residents in a manner and environment that provides dignity and full recognition of each residents individuality. The Licensed Practical Nurse will manage and supervise the delivery of services by CNAs and RAs. The Licensed Practical Nurse respectfully interacts with all residents, family members, visitors, colleagues, and staff. This is a safety sensitive position.

ESSENTIAL FUNCTIONS

  • Coordinate admission and orient new residents to the community, ensuring satisfactory transition for the resident and their loved ones.
  • Monitor and maintain records reflecting health and mental status of residents and reports any changes to the residents physician in a timely manner.
  • Administer treatment and records residents response.
  • Make recommendations regarding medication to physician and works with pharmacy to assist with the appropriate handling, dispensing, and destruction of medication.
  • Ensure that all resident rights listed in the Resident Bill of Rights are honored and respected by all staff members.
  • Execute infection control duties and ensure staff compliance.
  • Coordinate with dietary staff to ensure nutritional and therapeutic diets are served to residents as appropriate.
  • Investigates and documents any resident incident or issue.
  • Coordinate resident discharges and transfers.
  • Observe and assist in direct resident care.
  • Assist new staff with ongoing training.
  • Work within strict deadlines under potentially stressful conditions.
  • Participation in resident programming on daily basis
  • Regular and reliable attendance is an essential function of this job

 

ESSENTIAL MENTAL AND PHYSICAL FUNCTIONS

The essential mental and physical functions listed below are required in order for an employee to be able to perform the essential functions of this position.

  • Ability to read and write, follow written and verbal instructions, and communicate effectively in English.
  • Thinking critically; defined as reading, understanding, and taking appropriate action on detailed and complex information.
  • Walking long distances regularly.
  • Lifting and carrying up to 50 lbs.
  • Pushing a wheelchair with a resident weighing up to 200 lbs.
  • Repetitive actions/motion of one or both hands.
  • Hearing ordinary conversation and office sounds.
  • Verbal communication in person and on the phone.
  • Written communication by hand and on PC.

Qualifications

MINIMUM QUALIFICATIONS

  • Licensed Florida LPN; required to maintain licensure
  • Completed LPN Supervisory Course or ability to complete within 6 months of employment
  • Two or more years of related experience and/or training in geriatrics in long term care, home health, or other health care setting
  • Excellent verbal and written communication skills
  • Computer literacy which includes, but is not limited to, Microsoft Office suite
  • Outstanding communication and interpersonal skills
