Summary
The Healthcare Social Worker provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute or terminal illnesses primarily involving an adult population of disabled or elderly patients. Services include advising family care givers, providing patient education and counseling, partnering with care and case management on intervention and referrals to other services designed to promote health, prevent disease and address barriers in access to healthcare.
Essential Duties and Responsibilities
- Utilizing consultation data and social work experience, collaborates with case management team in delivery of quality care for patients with chronic, acute or terminal illness.
- Completes assessment of psychosocial needs or issues such as patient safety, housing, food, medication, transportation and other aspects of daily living by conducting home, hospital or care facility visits and consulting with patient, family members and other care givers.
- Identifies environmental impediments to patient progress through interviews and review of patient records.
- Matches and refers patient, family members and other care givers to community resources and services needed such as financial assistance, legal aid, housing and transportation.
- Assists with planning discharge from care facility to home or other care facility.
- Monitors, evaluates, and records patient progress according to measurable goals identified in treatment and care plan.
- Modifies treatment plans with changes in patient status.
- Follows through on objectives and adjusts plans to ensure quality and service goals are met.
- Empathizes and advocates for patients, family members or other care givers to resolve specific issues.
- Provides counseling to patients in individual and group sessions to help them overcome issues, dependencies, recovery from acute illness or adjustment to chronic conditions or palliative care.
- Organizes support groups or counsels family members and other care givers to assist them in understanding, dealing with, and supporting the patient.
- Investigates abuse or neglect cases and takes authorized protective action when necessary.

- Compiles information and prepares reports.

Education
Bachelor Degree in Social Work.
Experience
Three or more years of experience in healthcare social work.
Certificates, Licenses, Registrations
Valid driver license with appropriate insurance coverage and acceptable driving record.

Working Conditions
Work is performed in a medical office, hospital/care facility and home environments involving contact with patients and family members/care givers, physicians, staff and community resource partners. Regular local travel with use of personal vehicle and occasional evening hours are required.
