Physical Therapist Reports to: ADON/DON

Physical Therapists examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. He/she administers skilled care to clients requiring intermittent professional services and teaches the family and other members of the health care team. These services are performed in accordance with the physician's orders and the established plan of care, under the direction and supervision of the DON/ADON.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Observes and performs the initial evaluation visit to assess the consumer's condition for possible modification of treatment or initiation of additional medical procedures until the consumers treatment regime essentially stabilized. Identify and meet patients' goals and needs
  • Offer cost-effective treatments that help improve clients' motion and mobility Reduce the need for medications and provide alternatives to surgery
  • Develop care plans using a variety of treatment techniques
  • Create fitness and wellness-oriented programs tailored to patients' specific needs Provide quality, personalized and evidence-based care and proven interventions Motivate patients during treatment in order to help them function optimally Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination
  • Consult and practice with other health professionals as necessary
  • Evaluate effects, monitors and communicates progress throughout assignment via progress notes in a timely basis.
  • Communicates plan of care and progress with members of the health care team. Attends consumer care conference assigned to his/her care if requested/as necessary Participates in in-services programs and staff meetings as assigned
  • Works effectively during periods of increased work demands
  • Must be able to work effectively with other members of the health are team and consumer/family/significant others with varied ethnic, socio-economic backgrounds Must participating in continuing education activities to update skills, knowledge and ability.

Job Qualifications

EDUCATION and EXPERIENCE

  • Successful completion of a program approved by the National Board for Certification in Physical Therapy
  • Current Physical Therapy license issued by the state of PA Current CPR certificate
  • Compliant with state regulatory requirements. A valid license to practice physical therapy.
  • Excellent interpersonal communication skills. Compassionate and attentive to detail.
  • Possess up-to-date knowledge of treatment practices. Ability to solve problems independently.
  • Excellent analytical and logical thinking.
  • Ability to manage patients with different personalities.
  • Ability to write reports and correspondence at a professional level. Ability to use a computer and software proficiently.
  • Drive to continually learn new treatment techniques.
  • Proficient in technological devices/systems such as tablets, computers, laptops, e-mail, text messaging, and electronic documentation
  • Reliable transportation

One (1) year home health and clinical experience preferred.
