Description

  • Complete housing applications for all eligible youth in GH programs.
  • Develop and use a data base to track application due dates, expiration dates, renewal of psychiatric and psychosocial evaluations and other related appointment dates
  • Escort youth to all housing interviews and other related appointments
  • Monitoring CUCS data base to identify housing vacancies and report them to the case planning team
  • Interface and build partnerships with external housing programs such as NYCHA, NYNYIII and other alternative housing programs
  • Entering contacts and diligent efforts relating to employment and housing in connections as a collateral contact
  • Coordinating and executing housing plans with case planners and supervisors
  • Develops individual case service plans, using GH (Group Home) and HSVS guidelines and governmental objectives, and determines service needs, finds appropriate resources, schedules appointments for and escorts clients to resources as needed; enrolls clients in appropriate vocational programs, and monitors service delivery and progress.
  • Prepares and presents employment and housing service plans and updates to agency management, child care staff, clinicians and others as requested.
  • Participates in meetings, including interdisciplinary team, Family Team Conference, monthly child care/social services, weekly individual and group, and others as requested; occasionally prepares summaries of meetings to be documented in Connections related to employment and housing.
  • Collaborate with HSVS’ PYA departments to advance the employment strategic goals of our clients.
  • Attends appropriate training to remain knowledgeable of governmental employment and housing services.
  • Provide intense Independent Living Skill Workshop to clients 19 ½ Years old or older, to transition out of foster care with a goal of APPLA.
  • Facilitate a monthly open forum support group for older youth aging out of care to enhance independent living skill, financial literacy, obtaining and maintaining housing, independence and other topics related to maintaining stability after life in care. 
  • Work closely with case planning team to ensure all housing packets are submitted in a timely manner
  • Collaborate with guest speakers and influential individuals to create forums for older youth to meet with and obtain knowledge in preparation for the future 
  • Develop a transition timeline report for clients 19 ½ years old or older, to be reviewed by Supervisor and Director
  • Connect Clients to community resources prior to discharge
  • Provide 3 months aftercare support to clients who were discharge at 21 or older which includes bi/weekly call and in person meetings if necessary. (Clients may receive six months of aftercare support if deemed necessary this decision is made on a case by case basis)

Qualifications

  • Bachelors required, Masters Preferred 
  • Must have a valid NYS driver’s license
  • Must have excellent communications and writing skills
  • Must combine excellent interpersonal skills with strong leadership abilities.
  • Ability to interact effectively with multi-disciplinary staff and community partners
  • Must have the ability to maintain a flexible schedule when emergency duties or deadlines require it.
  • An understanding of and the ability to communicate the Agency’s mission
