Job Title: Behavioral Health Care Manager
Department: Mental Health Care
Location: Remote
Duties and Responsibilities:
- Perform behavioral health assessment and diagnostic evaluation
- Provide evidence-based behavioral interventions for patients
- Develop behavioral health treatment plans under direct supervision of psychiatric consultants
- Provide clinical and task supervision for behavioral health care coordinator
- Coordinate care for patients in the interdisciplinary team including primary care providers and psychiatric consultants
- Monitor and tract treatment engagement and outcomes
- Provide crisis intervention as needed
- Coordinate supportive care with family, community organizations or other agencies as needed
- Maintain accurate records in EMR system
- Attend case conferences, program meetings, and trainings as needed
Requirements:
- Valid LMSW/ LMHC license in NY state required
- 1+ years of experience providing evidence- based behavioral interventions in mental health clinic, preferred
- Strong communication and written skills
- Ability to work in fast paced environment
- Knowledge of and experience in EMR system.
- Bilingual English and Chinese (Mandarin or Cantonese) required
