Location: Brooklyn and Flushing, NY
Position: Patient Care Navigator
Duties and Responsibilities:
- Facilitate communication between patients and physicians
- Conduct initial screening and intake assessment
- Provide advice, support, and direction to medical receptionists
- Track medical records and maintain documentation of all patient encounters
- Assist physicians to complete patient visits in a confidential manner
- Address patient inquiries under physician guidelines
- Coordinate patient care with medical staff, family members, social service organizations, other health care providers and facilities
- Obtain prior-authorizations for medications as needed
- Other duties as assigned
Requirements:
- Bachelor’s degree preferred
- Bilingual in English and Chinese (Mandarin or Cantonese) required
- 1+ year of experience working in a mental health clinic preferred
- Strong customer and patient services skills
- Strong interpersonal and organizational skills
- Ability to work under pressure and able to multi-task
- Strong communication skills both in written and verbal forms
- Caring and compassionate personality
