SOC Care Coordinator

We are seeking a SOC Care Coordinator to join our team!

Responsibilities: 

• Schedule, facilitate, and completes all paperwork for Wrap Services.
• Completes all SOC assessments (initial, updates, and transitions).
• Link, advocates, refers families to resources.
• Completes YIS Service entries.
• Advocates for youth and family to schools and other providers.
• Monitors progress with youth and family according to the treatment plan.
• Timely and accurate completion of progress notes.

Requirements: 

• High School diploma or equivalent. Bachelor’s degree in Mental Health, Social Services, or Human Services required.
• Certified Case Manager II certification required, or willing to get. Must keep current and maintain CEU requirement.
• 1+ year experience working in social services or mental health field.
• BLS CPR certification.
• Wrap 101 training required.
• Current Oklahoma driver’s license with clear DMV background check. Must provide proof of vehicle liability insurance.
• Intermediate computer skills and proficiency with electronic health records software as appropriate.
• Must be a self-starter, multi-tasker, with ability to effectively give direction and input to care team.

WORKING CONDITIONS

When in office must be able to sit in a chair and maneuver the body while sitting and using a computer keyboard. Must be able to carry 15 lbs. when in normal office environment.  Will drive to and from client’s homes or places of service. Home visits will require exposure to clients’ home environment.
