Job Description:

This position will assist Teleneurologist with daily rounds, assist with neurological examination of patient, operations and coordination of patient care and outcomes, documentation in EMR, order entry, and gathering of documentation as needed to maintain a complete Medical Record. The duties include performing clerical and information technology functions in the hospital setting. communications with patient, patient family and hospital medical staff. The ideal candidate will maintain clinical and professional competency as appropriate to the population of patients served.

This position will complete rounds on inpatients to improve quality, safety and reduce operational risk exposure through performance improvement processes while meeting regulatory and accreditation requirements.

Performs all duties and responsibilities in accordance with the established FMC policies, procedures and guidelines.

 

  • Collect nursing assessment data for communication to the physician, patient/family, and or nursing staff for facilitation of Rounding.
  • Assist Teleneurologist with patient rounding and neurologic exam.
  • Assist in use of Telemedicine equipment in rounding.
  • Assists in identifying expected outcomes to provide direction for continuity of care.
  • Communicates and collaborates effectively with other health team members regarding patient situations and actions needed.
  • Visit patients as directed by the supervising provider for the purpose of collecting data through chart review, interview, and observation.
  • Review medical, nursing and multidisciplinary data in the medical record and communicate findings to the physician.
  • Record, aggregate, and document results of relevant systematic observations, diagnostic and therapeutic tests in the EMR.
  • Clarify and answer questions for patients/families and nursing staff about the supervising physician’s medical plan of care.
  • Rounds on patients to ensure quality metrics are followed.
  • Serves as a change agent by collaborating with other departments, frontline staff, and others to influence change and the improvement of quality and safety.
  • Works closely with all areas and key stakeholders to ensure complete, efficient, and integrated projects and core responsibilities.
  • Maintain communication with QE team members to coordinate continuity of care by participating in daily huddles, staff meetings, data review, and other collaborative events.
  • Other duties as assigned.

 

Qualifications

Job Qualifications:

  • Associate Degree in Nursing, Bachelors preferred
  • Valid license to practice nursing in the State of Ohio
  • Hospital care provider experience preferred
  • Must achieve NIH certification
  • Variable shifts including weekends
  • CPR required
  • Previous nursing experience preferred

Must be able to physically exerting 50 to 100 pounds of force occasionally and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for Medium Work. Position may require frequent patient handling activity.
