POSITION PURPOSE/SUMMARY: As a member of the multi-disciplinary team the caregiver/CNA will provide personal care services to the terminally ill patient/family in accordance with a written plan of care.

CLINICAL SERVICES: Responsible to the LVN on duty, and/or the Hospice House Administrator for carrying out the duties assigned in order to increase the level of comfort, and to maintain personal hygiene and a safe, healthy environment for the patient.
1. Services are carried out according to Hospice of San Joaquin's policies.
2. Provide or assist patient in bathing, care of mouth, skin, or hair.
3. Assist patients in and out of bed and assist with ambulation.
4. Turn patients in bed and positions for comfort.
5. Assist with prescribed exercises which patients and Health Aide Caregivers/CNAs have been taught by appropriate health personnel.
6. Prepare and serve meals and assist patients with eating.
7. Assist patients to the bathroom or in using a bedpan or urinal.
8. Give enemas as ordered and instructed.
9. Perform household services that will facilitate the patient’s self-care at home as necessary to prevent or postpone institutionalization.
10. Change bed linens.
11. Take and record vital signs as directed.
12. Complete appropriate records.
13. Adheres to all Hospice of San Joaquin policies, and procedures, including following Hospice of San Joaquin infection control procedures and the code of ethics.
14. Other assignments may be appropriate.
