Nurse Practitioner/Physician Assistant in Women Health

PURPOSE OF POSITION

For over 50 years, Covenant House New York (CHNY) has been a leader in providing a full continuum of shelter, transitional and permanent housing and wrap-around support services for vulnerable homeless, runaway and exploited youth. As the largest provider of runaway and homeless youth services in New York City, CHNY serves more than 1,500 youth and young adults ages 16-24 annually. Our doors are open to all young people who need help, regardless of their race, religion, sexual orientation, gender identity or expression. We provide shelter, housing, education, employment, health, mental health and other services using harm reduction and trauma-informed principles. We also advocate for systemic change through public policy while promoting awareness of critical issues related to youth homelessness. Covenant House New York’s goal is to help each young person find safety and refuge from the dangers of living on the streets and to empower youth now and into the future. The Nurse Practitioner/Physician Assistant, under the direct supervision of the Medical Director, provides women’s health services and primary care to our Health & Wellness center patients using a model that focuses on holistic patient care, including health promotion and behavior change

 

DUTIES AND RESPONSIBILITIES

(Responsibilities to include but not limited to the following)

  • Provides age-appropriate primary and women’s health patient care.
  • Cultivate a climate of trust and compassion for the patients.
  • Elicits, records, and interprets the past medical history, family, and social history as it applies to the resident.
  • Functions as a member of an interdisciplinary team to plan, deliver and evaluate health care for a defined patient population.
  • Orders and performs necessary diagnostic tests, procedures and therapeutic interventions as identified in the plan of care. These procedures are to be according to established protocols in which the NP/PA is trained and approved to implement.
  • Collaborate with the provider staff and administration to monitor and evaluate the quality of patient care.
  • Determine care requirements and develop programs for a specific patient population, regarding patient condition and age, and follow universal precautions.
  • Promote and support patient health maintenance that spans the continuum of health from outpatient to inpatient settings and back to the home care arena.
  • Perform medical diagnostic, therapeutic procedures, and prescribe, dispense or deliver medications according to Covenant House protocol, collaborative practice agreement.
  • Provide anticipatory guidance and counseling to patients' families.
  • Completes timely documentation of patient assessment and plan of care in the electronic health record and generates appropriate health reports for outgoing correspondence.
  • Initiate and coordinate referrals to other health care team members or agencies.
  • Serve in a consultative resource capacity by providing expert knowledge and behavioral skills that impact clinical practice and patient outcomes.
  • Participates in the development, utilization, and evaluation of resources to meet patient/family educational needs.
  • Provide health promotion education and serve as a resource for nursing staff in areas of clinical expertise.
  • Participates in community events to promote the mission of the Health Center.
  • Generates reports regarding health care for particular patient populations.
  • Incorporate best practices standards as a Nurse Practitioner in the care and treatment of all patients.
  • Participates in development and implementation of divisional and departmental quality assessment/improvement/health services evaluation programs and processes.
  • Responsible for administrative activities and special projects as assigned.
  • Contributes toward the achievement of divisional and departmental goals.
  • Participate and contribute to activities that enhance clinical expertise and nursing knowledge.
  • Knowledge of Current Procedural terminology coding (ICD-10 codes) as required by third party payors.
  • Maintain effective working relationships with the supervising physician and other members of the health care team.
  • Serve as a member of Health & Wellness Center committees as assigned.
  • Follows Health & Wellness Center policies, practices, and standards of care set by City, State, and Federal regulations.
  • Provides direction to medical students, interns, or residents assigned for training.
  • Prior experience with electronic health record systems.
  • Demonstrate and maintain the standards and requirements of the Health Insurance Portability and Accountability Act (HIPAA).
  • Perform other duties as assigned.

 

SUPERVISORY DUTIES

  • This position does not supervise any other position.

Qualifications

KNOWLEDGE, SKILLS, & ABILITIES

Education & Experience

 

  • Master of Science in Nursing (MSN) degree or higher from an accredited program/graduation from an accredited physician assistant graduate program.
  • Active and unencumbered Nurse Practitioner/Physician Assistant license.
  • Must possess active Drug Enforcement Agency (DEA) registration or be eligible for registration.
  • Have a minimum of one (1) year experience as an NP/PA in one of the areas noted.
  • Must function according to standing orders developed in consultation with the Medical Director/Clinic Physician.
  • Must possess active Medicaid Enrollment or be eligible to enroll.
  • Comfortable with Electronic Medical Record (EMR - eClinicalWorks) system and Microsoft Office.

Knowledge/Skills

  • Demonstrates the ability to work effectively with people from culturally diverse backgrounds, and be able to display age-specific competency.

  • The ability to manage time well and work in a fast past ambulatory health care environment.

  • Excellent written and verbal communication skills are required.

 

WORK ENVIRONMENT/ PHYSICAL DEMANDS

  • High concentration.

  • Frequent interruptions.

  • Moderate noise level.

  • Frequent standing, walking, bending and lifting (up to 20 lbs.).

  • Frequent sitting for periods of time at a computer workstation.

 

Schedule 9:00 am - 5:00 pm

Salary range is $130k - $140k

Compensation

Covenant House New York offers a rich package of benefits including but not limited to full medical, vision, dental, life and short and long-term disability insurance and other fringe benefits. Eligibility for medical benefits begins on the date of hire. PTO includes 3 weeks of vacation in the first year, 10 holidays, 13 sick days, 5 personal days and birthday days off. Benefits include a 403b with employer contribution after 12 months.
