Job Summary:
As a vital member of an interdisciplinary team, the Peer Navigator (SUD Recovery) uses his or her lived experiences of recovery, plus skills learned in formal training, to deliver services to individuals seeking a treatment referral for Medication Assisted Treatment (MAT) services. Engages clients into services and/or programs aligned with their recovery by fostering connections between the client and their treatment team. Collaborates with the local communities to promote opiate crisis awareness and MAT services. Assists with the distribution of Narcan.
Essential Functions:
- Serves as a peer mentor/role model by using his or her lived experience of recovery, plus skills learned in formal training, to deliver services to individuals seeking a treatment referral for Medication Assisted Treatment (MAT) services. Shows compassion, dignity and respect; possesses active and reflective listening skills; has a clear sense of boundaries; and is far enough in the recovery process that they can manage job difficulties without compromising their own personal wellness and recovery.
- Assists with outreach and peer-based services through the promotion of hope, responsibility, empowerment, and self-sufficiency.
- Engages clients into services and/or programs aligned with their recovery by fostering connections between the client and their treatment team. Establishes and maintains weekly communication, either on site or by phone, with individual Opioid Treatment Providers (OTP), MAT providers and hospital Emergency Departments regarding service availability. Supports individuals referred by any of these identified points of contacts, or by other routes, with admission into an evidence-based, MAT treatment program within their region (must facilitate a minimum of five treatment referrals each month that result in a treatment admission for an Opiate Use Disorder to identified providers.)
- Coordinates care for clients entering residential treatment.
- Directs clients to outpatient treatment and sober living services.
- Aides in the completion of consumer forms within the scope of position.
- Maintains current and cultivates new relationships with community agencies.
- Participates in regular community meetings focused on addressing opiate challenges.
- Performs monthly outreach efforts to harm reduction organizations or other entities engaged in naloxone distribution, including withdrawal management facilities.
Knowledge/Skills/Abilities:
· High school diploma or equivalent, required.
· Bachelor’s degree in Human Services or related field, preferred.
· Self-identified consumer in recovery from addiction with a demonstrated ability to maintain personal wellness and recovery tools.
· Peer Specialist Certification through IC & RC (within 6 months of hire).
· Mental Health First Aid Training (within 60 days of hire).
· Working knowledge of community resources.
· Excellent customer service skills.
