Description

Safe Haven’s provide treatment and shelter to adult men and woman in need of support. Safe Havens operates similar to homeless shelters but are more private and less restrictive due to serving a population that has been exposed to longer-term street homelessness. Clients address their barriers at their own pace with the support of the staff on-site. Safe Haven’s serves those with serious mental illness and often co-occurring substance use disorders.

 

 

This is a five-day-per-week position with a flexible schedule, within established boundaries.  It requires the individual to work at multiple sites in a shared office, coordinating their schedule with other medical staff.  In collaboration with the clinical team and under the Supervision of the Program Director, this position works with homeless adults who are diagnosed with serious behavioral health disorders, and/or those who may have a co-occurring substance abuse disorder.  The Bridge has a strong commitment to providing evidence-based and best practices that emphasize health and wellness throughout its continuum of outpatient, community support and residential services.   The Licensed Practical Nurse (LPN) is an integral member of a multi-disciplinary team.  This position requires a highly organized, detailed, and assertive individual who is flexible and willing to work in a high-demand environment.  The position requires direct care work assisting clients with minor medical issues and advice, referrals, and performing training for both staff and clients.

 

 

Essential Position Functions:

 

  • Assist consumers in accessing medical care.
  • Assist the program in monitoring the medication of a portion of the population, including performing quality assurance checks of documentation and opening a rapport with the program’s pharmacies.
  • Monitoring vaccinations and health initiatives.
  • Work in conjunction with the Clinical staff at all facilities in conjunction with Managers, Supervisors, other program staff, the Psychiatric Nurse, and the Clinical Coordinator.
  • Perform related administrative duties, including correspondence, reports, and record-keeping. 
  • Document direct and indirect contact with consumers in the electronic documentation systems that include AWARDS and CARES reportable to DHS.
  • Make and follow up on referrals and act as a liaison with outpatient providers, inpatient hospitalizations, and emergency rooms when indicated on a case-by-case basis.  his includes visits to inpatient clients and/or outpatient escorts as needed.
  • Provide educational activities for consumers on health-related issues and co-lead staff training on medication monitoring.
  • Assist with any crisis activities that may occur while on duty. Report any client incidents or disruptive behavior.
  • Attending meetings, conferences, and training, as required.
  • While performing functions, one must be able to interact with the client population politely and professionally while maintaining personal boundaries. 
  • Must be able to and being able to accept our resident’s behavior and maintain composure during crisis situations and/or engage individuals who may present behavioral issues.
  • Must be willing to travel between locations and flexible with regard to duties, location, and schedule. Willing to visit with clients, in the facility or outside the facility, in locations other than the office.
  • Any other duties may be assigned by any management employee in the facility.

 

 

 

 

Qualifications

Required Knowledge, Skills, and Abilities:

  • License and current registration to practice as a practical nurse in NYS.
  • Three years of experience working with individuals with mental illness and/or co-occurring substance use disorders.
  • Experience in a culturally diverse environment and experience understanding various evidence-based models and theories pertaining to psychoeducation, motivational interviewing, mental illness, and recovery.
  • Highly organized and self-motivated. Proficient in computers.
