About the Program 

The Family Treatment Rehabilitation Program (FTR) offers intensive preventive case management services to families at risk of removal of children to foster care due to neglect and abuse. Risk factors for families served by the FTR program are sometimes advanced as a result of caretakers’ drug use and/ or mental illness. The stressors of poverty, substance use, instability and criminal justice involvement can greatly compromise a parent’s capacity to care for her children, despite strong desires to be engaged. Our goal is to partner with families to keep them together while ensuring that children will be safely cared for.

The Case Planner in our FTR program provides assessment, engagement, case management and other support services which help ensure that children have strong families to grow up in and have the preparation and opportunity to succeed in school and life. Duties and Responsibilities:

  • Creates, in partnership with families, service plans which address safety, behavioral, educational, and psychological and permanency needs.
  • Monitors safety, well-being and progress against other critical outcomes by facilitating regular casework contacts and by documenting the same in progress notes and other case documentation.
  • Collaborates with internal and external service providers and other stakeholders through clear and timely verbal and written communication.
  • Participates actively in Family Court Hearings, Family Team Conferences and other meetings to advance permanency for children and families.

 

Salary & Compensation:

  • Salary: $55,000.00 to $58,000.00/annually + Potential to Earn Performance Based Merit Award for Eligible Staff + Comprehensive Benefits + Professional Development Opportunities.
  • FLSA Status: Non-exempt, position is eligible for overtime.

Qualifications

Qualifications:

  • Bachelor’s Degree in Social Work or related field
  • Bilingual Spanish speaking required
  • Critical Competencies: Detail-oriented
  • Excellent verbal and written communication skills
  • Flexibility, professionalism and ability to function as a team player and possesses a demonstrated passion for advancing the work of serving children and families
  • Ability to work with diverse populations
  • Familiarity with NYS Connections system a plus
