Perform outreach at homeless camps, shelters, emergency departments, detox centers, and other locations, with priority given to pregnant and parenting women, IV drug users, homeless individuals and those involved with the criminal justice system. These services are provided pre-enrollment to MAT.

Build and maintain professional relationships with local service organizations, hospitals, police, jails, courts etc.

Provide individuals with appropriate referrals to community programs and services.

Provide follow up on all referrals to assess outcomes and additional services as required.

Conduct initial screening of individuals for eligibility for MAT services and makes appropriate referrals.

Coordinates intake and enrollment with the appropriate CODAC staff by supporting individual until enrollment is completed.

Performs integrated healthcare assessments.

Assess and addresses barriers to treatment engagement.

Track and maintain written records of community contacts and outreach for potential enrollments.

Inform and educate members regarding the meaning and implications of test results and prescribed interventions, as per clinic protocol.

Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.

Provide support and/or assistance to other departments to ensure service engagement for MAT.

Assists in wellness and reengagement outreach via telephone, home visits, jail visits, and other viable means.

Transports prospective members to site for intake and to local service organizations, as appropriate.

Provides crisis intervention and de-escalation of members, as needed.

Attends all staff meetings and scheduled trainings.

Performs other related duties in accordance with agency growth and changes.
