Golden Heart Home Services, LLC is seeking a self-starting, autonomous Licensed Practical Nurse (LPN) to join our growing team in the Flint area. This is an excellent opportunity to supplement your income with a flexible schedule while making a meaningful impact in home care.
Position Highlights:
Responsibilities:

Supervise and train CNAs and DCWs.

Perform supervisory home visits every 90 days.
-
Conduct skill check-offs for new staff.
-
Review and update service plans every 90 days.
-
Fill medication reminders on a weekly and bi-weekly basis in clients' homes.

Apply medication patches and document all skills in the EMR system.
-
Report directly to the RN Administrator.
Requirements:
-
Must have experience in home care/home health.
-
Trach and vent experience required.
-
Must carry professional liability insurance.
-
Ability to purchase workers' compensation insurance.
-
Must be reliable.
-
Valid driver's license and vehicle insurance.
-
Current LPN license in the state of MI.
-
Self-starter with excellent communication skills.
-
2 years of experience in home care, home health, LTC, and/or hospice.

Comfortable learning to chart on an EMR system.
-
Compassionate and knowledgeable.
-
Current CPR/BLS/First Aid certification.
-
Current TB test.
-
Able to pass a criminal background check.
