Callen-Lorde Community Health Center seeks a dedicated, flexible individual to join our staff as a Case Manager of Care Coordination services. The Case Manager will be responsible for supporting, assisting, and ensuring that clients’ biopsychosocial continuum of care needs are identified, and services are appropriate and monitored. The Case Manager will facilitate patients’ transition into primary medical care, and work closely with a medical, nursing, and behavioral health care team to ensure appropriate linkage to supportive services, enrollment in programs to cover cost of health care, as well as retention in care. This position works with a caseload of patients as well as provides services on a walk-in basis. The Case Manager works with any Callen-Lorde patient, both in the office as well as in the field as needed. In this role you will provide intake, needs assessment, clinical care orientation and ongoing reassessment services to patients and their significant collaterals, documenting all such services in the medical record. You will also, Ensure enrollment into programs offering a higher level of care for eligible patients, as needed. Callen-Lorde is typically open Monday through Saturday. Weekly evening and occasional weekend hours will be required.
