Description

Provide direct care to adults with Significant Mental Illness that impact functioning in multiple areas of their lives. Complete intake paperwork, functional assessments, and individual treatment plans and required 6-months review documents to provide ongoing services. Provide home and community based mental health rehabilitative services to clients in the form of support, coaching, and educating clients for the purpose of promoting stability, social functioning, and personal and emotional adjustment and independent living skills.

  • Maintain caseload of 15-25 clients
  • Provide in-home and community base services.
  • Ability to provide culturally sensitive services.
  • Develop trusting and therapeutic relationships with clients.
  • Complete documentation. assessment, Individual Treatment Plans, Progress Notes and other required documentation.
  • Monitor and evaluate clients’ progress regularly, adjusting treatment plans as necessary.
  • Work collaboratively within the Behavioral Health department, and medical providers to meet the needs of clients.
  • Coordinate services with other case managers, mental health providers as needed.
  • Participate in clinical and administrative supervision of cases through individual supervision, team meetings, and training.

Work Environment

The primary environment is an ambulatory clinic; secondary environments: client homes, and community. This role requires regular walking. This role may come into contact with patients who may have contagious illnesses.

 

Physical Demands

  • Prolonged periods of sitting at a desk and working on a computer.
  • Requires walking and standing.
  • Must be able to lift 15 pounds at times.

 

Travel Requirements

Must have the ability to travel between MCC sites in the St. Paul/Minneapolis metro area and clients’ homes and communities.

Qualifications

Required Education and Experience

  • Master’s Degree in one of the behavioral sciences or related field OR current master’s degree student OR a bachelor’s degree in of the behavioral sciences with a minimum of 2000 hours of supervised experience in the delivery of mental health services to adults with mental illness OR fluent in the language of the cultural group at least 50% of the practitioner’s clients belongs.
  • Licensed in related field or LGSW/LICSW preferred


Additional Eligibility Requirements

  • Demonstrated success in working effectively with target population(s).
  • Change Agile; ability to operate in the gray and flex to new developments or situations.
  • Experience working in a multi-site environment is highly desired.
  • Must have valid driver’s license and access to a reliable vehicle with insurance coverage.
  • Experience working with individuals with serious mental health illness or similar populations.
  • Must feel comfortable traveling to clients, meet them at their homes or community base.
  • Proficient in record-keeping and documentation
  • Strong and effective written communication skills
  • Ability to work independently and as part of a team.
  • Must have basic technology skills.
