SUMMARY

In accordance with RNP's Mission Statement, Philosophy of Client Care, and Code of Ethics as stated in the Personnel Policies and under the supervision of the Department Director, provide general direction and supervision to departmental staff and manage an assigned caseload of clients. Provide Rapid Rehousing case management to clients impacted by gun and gang violence and the criminal justice system who are literally homeless or experiencing housing instability and may have challenges with substance abuse and co-occurring mental health issues by performing the following duties.

 

CORE FUNCTIONS

  • Supervises staff by utilizing appropriate management techniques, including day-to-day direction, scheduling, training, crisis management, planning and evaluation.
  • Assist with daily operations of PHS.
  • Maintains a monthly supervision log as evidence of clinical and administrative supervision.
  • Works directly in treatment relationship with identified program participants.
  • Provide Rapid Rehousing case management up to 20 PL households
  • Conduct home visit.
  • Conduct unit inspections.
  • Provide education to clients regarding housing stability.
  • Works in partnership with Project Longevity (PL), Adult Probation, Court Support Services Division (CSSD), Career Resources and Department of Housing (DOH).
  • Maintains records and complete reports as required by funding sources.
  • Facilitates weekly Rapid Rehousing case management and team meetings. Maintains meeting minutes.
  • Works in close collaboration with physicians and other health care personnel in the provision of quality care to all persons served.
  • Reviews and audits charts monthly.
  • On-call duties.
  • Assists the department director in long range planning and quality improvement.
  • Assumes all other appropriate duties as deemed necessary by the supervisor.

Qualifications

CORE COMPETENCIES

Bachelor's degree in the behavioral health field and two (2) years experience in counseling, case management or rehabilitation services preferred; or equivalent combination of education and experience.  Co-occurring competencies and/or credentials are a plus. Two years supervisory experience preferred.

  • Knowledge of principles and techniques of counseling, crisis intervention, and/or case management. 
  • Understanding of and ability to provide integrated care for individuals with co-occurring disorders
  • Ability to gather, analyze and evaluate information pertinent to the care of persons served. 
  • Knowledge of the physical, psychological, social and economic effects of substance abuse, homelessness, and mental illness. 
  • Knowledge of community resources. 
  • Ability to develop relationships and maintain a resource listing of Landlords, Property Management companies, and Realtors to assure Rapid housing solutions for  participants.
  • Considerable knowledge of federal and state laws and regulations regarding client confidentiality. 
  • Ability to establish and maintain cooperative professional relationships. 
  • Excellent communication skills. 
  • Ability to project a professional image.

 

CERTIFICATES, LICENSES

Incumbents may be required to secure professional certification and/or license.  Valid Connecticut driver's license may be required.
