POSITION OVERVIEW:
The Case Manager will receive one hour of individual supervision per week, and take part in a weekly training meeting consisting of one hour of group supervision, didactic training, and staff meeting. It is possible for the case manager to accrue supervised clinical hours required for license as a MFT, LCSW, or Psychologist. Essential Job Functions and Responsibilities:
- Perform regular assessments of clients' needs in the following areas: housing, daily activities, counseling, coordination of psychiatric care, social relationships, family relationships, symptom management, medical and dental care, securing and managing entitlements such as SSI, budgeting, and other related areas.
- Develop collaborative treatment plans with clients to help them accomplish activities, goals and objectives. Implement the plan through the provision of services, referrals and coordination with family and other community service providers.
- Assist clients to find appropriate housing at local shelters, board and care homes, transitional housing and independent living placements in the community.
- Assist clients in applying for entitlements and managing entitlements.
- Attend weekly staff meetings, didactic seminar and case conferences.
- Assist clients with locating and accessing needed health care and dental care.
- When necessary, conduct home visits and attend appointments with clients.
- Assess client’s transportation needs and help them access transportation support. This may include driving clients to appointments, helping them apply for bus passes, para-transit, and other types of transportation assistance.
- Collaborate with staff of local hospitals, board and care homes, shelters, day treatment programs, and other agencies in a friendly and professional way.
- Learn about Alameda County social service resources, housing resources, community resources and build relationships with resource providers.
- Maintain timely documentation for clinical services as required by funding sources.
- Regularly review files and monitor activities of clients with follow-up contact in-person or via phone.
- If accruing hours toward licensure, the case manager has some opportunities for providing individual counseling, family therapy and group therapy.
- Assist with intakes and officer of the day crisis coverage as needed.
- Maintain a full caseload of clients. Most clients will be adults who have a serious pervasive mental illness.
- Other relevant duties as assigned or required.
- Provides clinical and case management services to BHCS clients; interviews clients and family members; conducts initial and ongoing psychosocial strength-based assessments.
- Develops initial and ongoing treatment plans on assigned clients; identifies and pursues necessary resources to implement treatment plans.
- Conducts individual, group, and family psychotherapy sessions; provides education on behavioral health care symptoms and the role of medication in alleviating such symptoms; diagnoses illnesses and dual recovery/co-occurring disorders.
- Prepares and maintains psychosocial case histories for use in diagnostic evaluations and monitoring client progress.
- Provides crisis intervention as needed; stabilizes clients in crisis including brief intervention in clinic; arranges for hospitalization and coordinates treatment elements.
- Maintains all records in accordance with the Quality Assurance standards of Alameda County Behavioral Health Care Services.
- Participates in clinical case presentations and peer reviews.
- Prepares reports, case summaries, correspondence and other written materials.
- Testifies in court when required for determination of client's legal status regarding outpatient court-ordered treatment.
- Conducts mental status examinations and formulates Diagnostic and Statistical Manual (DSM)-IV related diagnoses.
- Collaborates with other departments and agencies related to client treatment planning.
- Provides education and consultation services to Alameda County departments and the community as assigned.
- May provide direction and assistance to Behavioral Health Clinicians I and graduate interns.
- May serve as a liaison to other community agencies
