Description

Utilizing their knowledge of the healthcare industry, the Health Career Mentor connects clients to healthcare programs and works with clients to reduce barriers to enrollment to support economic mobility. The Health Career Mentor is also responsible for completing needs assessments and vocational evaluation in order to support program participation in employment preparation and career advancement strategies.

RESPONSIBILITIES 

Employment Services:

  • Determine referred client’s skills, work preferences, and individual needs through screening and assessments. Complete vocational goal plans and success plans.
  • Stay up to date regarding healthcare industry trends, available certifications and degree programs and the related labor market. 
  • Maintain partnerships with outreach/enrollment staff of healthcare training/education programs.
  •  Communicate individually with team members to coordinate services and generate ideas to help ForKids’ clients achieve their employment goals and address job retention to include conducting mock interviews.
  • Work collaboratively with Family Case Managers to ensure that clients understand the impact of earned income on their benefits (i.e., SSI) and make referrals for benefits counseling as needed. 
  • Assist clients in resolving and mitigating barriers to obtaining and maintaining employment during the vocational goal setting process including teaching conflict resolution strategies. 
  • Assist clients in making a childcare and transportation plan as needed, offering resources that may be available to assist with the cost.
  • Work collaboratively with client to address any barriers to vocational program completion that arise during the program.
  • Complete assigned data collection within electronic case management system and participate in summarizing and analyzing data trends and identifying programmatic changes consistent with knowledge gained from data collection. 
  • Participate in program development and planning activities in relation to employment and economic mobility. 
  • Assist client with obtaining employment in their designated field upon completion of training program.
  • Check-in monthly with clients participating in healthcare training programs, helping to problem-solve through any challenges and make a plan for securing employment. 

Customer Service:

  • Provide service to internal and external customers according to standards as outlined in ForKids Customer CARE Manual. 

Financial Management:

  • Use agency resources (financial and non-financial) prudently.
  • Acknowledge and follow financial policies of the agency.

Grants Data Management:

  • Accurately complete all required data reporting in accordance with established guidelines.
  • Participate in all scheduled data collection trainings.

Community Engagement:

  • Participate in development and fundraising activities as needed and requested (ex: meeting with donors, providing tours, attending fundraising events).
  • Regularly engage, support and collaborate with volunteers to provide a meaningful experience.
Qualifications

Education and Experience:

Graduate of a healthcare training program with supervisory or training experience.  Experience with low-income or vulnerable populations preferred. 

Essential Functions, Knowledge, Skills and Abilities:

  • Experience with crisis intervention, community networking and resource development.
  • Experience working with adult learners.
  • Proven ability to present material, communicate clearly, and effectively engage with participants in classroom settings and individually.
  • Excellent interpersonal skills and ability to work with diverse populations.
  • Excellent written and verbal abilities.
  • Ability to work under pressure to meet targets and deadlines.
  • Ability to work in challenging environments.  
  • Knowledge of social, economic and health problems of those living in poverty.
  • Knowledge of available community services and programs.
  • Ability to drive the agency’s 12-passenger vans.
  • Valid driver’s license with no more than three minor moving violations and/or accidents combined within the last 3 years, and no major moving violations in the past five years.
