Job Summary:
This position is responsible for providing mental health services to clients with intellectual and developmental disabilities and behavioral/mental health disorders. The Licensed Clinical Social Worker will work at the discretion of the interdisciplinary team as a contributing member to assess, diagnose, and contribute to individualized habilitation plans to improve clients' emotional well-being, behavioral health, and overall quality of life.
Key Responsibilities:
- Conduct Assessments: Perform social evaluations and assessments of clients to determine their mental health treatment needs, in accordance with CMS and state regulations and facility policy.
- Contribute Individualized Habilitation Plans: Create person-centered treatment plans based on assessment findings that comply with CMS, state, and facility requirements for Individualized Habilitation Plans (IHPs). Ensure plans address the emotional, psychological, and social needs of each client.
- Communicate with the Team: Provide Goals/Objectives to the Team based on individual assessments and communicate progress toward those Goals/Objectives to the Qualified Intellectual Disability Professionals (QIDPs) on a monthly basis.
- Provide Therapy and Counseling: Deliver evidence-based therapeutic interventions, including individual and group therapy consistent with training and experience, to promote emotional regulation, adaptive coping strategies, and social skills development.
- Crisis Intervention: Provide timely crisis intervention services, including de-escalation and emergency therapeutic interventions as approved by the IDT.
- Collaboration with Interdisciplinary Team: Collaborate with QIDPs, Board Certified Behavior Analysts (BCBAs), psychologists, physicians, nurses, direct support professionals, guardians, family members, and other team members to ensure mental health services are integrated with the overall care plan.
- Family and Caregiver Support: Engage and consult with family members, guardians, and caregivers to communicate progress, provide education, and ensure continuity of care at home and in the community.
- Regulatory Compliance: Ensure all services and documentation comply with the Oklahoma State Board of Licensed Social Workers and CMS regulations, including maintaining accurate and up-to-date records that reflect the care, treatment, and progress of clients.
- Training and Staff Support: Provide training and support to staff on mental health issues.
- Monitoring and Documentation: Maintain accurate and timely clinical records in accordance with facility policy, ensuring compliance with privacy and confidentiality requirements (e.g., HIPAA). Regularly evaluate and adjust treatment plans based on the progress and ongoing needs of the clients in accordance with the IDT.
- On Call: Remain available on a designated on-call schedule, often responding to phone calls or emergency visits during evenings, weekends, and holidays.
- Follow-Up: Provide follow-up care or referrals to ongoing treatment providers when necessary.
- Other duties as assigned.
Physical Demands:
- Ability to stand, sit, bend, lift, climb, balance, stoop, kneel, crouch, and move intermittently. Use hands to handle or feel. Ability to turn on the knees, back, wrist, shoulders, and grasp with both hands. Must be able to frequently lift/move up to 50lbs. and occasionally lift and move more than 50lbs.
