Callen-Lorde is seeking a dedicated and flexible individual to join our staff as a Case Manager (CM). The CM is responsible for providing case management, treatment education, and counseling services to a diverse patient population, including people of transgender/gender non-conforming experience and HIV-positive individuals accessing medical and mental health services. The CM will work closely with medical, mental health, and outreach workers to ensure that the full continuum of care needs of our clients are identified and appropriate services accessed, coordinated, and monitored. The Case Manager will work full time at our Brooklyn clinic. Currently, due to the pandemic response, services are being provided both onsite and remotely, so this position will have to work at the Brooklyn clinic location as well as work from home to provide patient care.
Callen-Lorde’s Brooklyn site is open Monday through Saturday. This is a full-time position with morning, afternoon, and evening hours.
