Description

We are looking for a responsible, dedicated, and enthusiastic individual to work as our San Diego Direct Service Coordinator. This is a full-time position and requires a special aptitude for accuracy, confidentiality, organizational skills, management skills, and interpersonal skills. 

 

AS THE DIRECT SERVICE COORDINATOR YOU ARE  responsible for but not limited to performing the following essential job duties:

  • Advance client’s decision-making, productivity and participation in a range of daily activities. Facilitate the client’s inclusion in positive community experiences. 
  • Promote independence in daily activities yet be mindful of client’s intellectual and physical disabilities and assist with, meals, cooking, household maintenance, grooming and dressing, as assigned. Maintain current knowledge of client’s needs.
  • Implement individual training and support programs under the direction of the case responsible person.
  • Maintain positive communications and relationships with his/her supervisor, clients, landlords, employers, involved members of families, Regional Center, D.O.R. and other individuals in the community.
  • Participate in the evaluation of the client through completion of task analysis and other requested assessment devices.
  • Document required activity records, reports, incidents, and client evaluations in a timely, complete, and accurate manner as required.
  • Provide positive advocacy when the individual’s preferences, needs, or talents are neglected or overlooked.
  • Address challenging behaviors proactively and respectfully, using methods outlined in plans and training.
  • Oversee client activities and environment throughout shift to ensure the fulfillment of health and safety needs.
  • Assist with training employees to perform their direct service duties in accordance with the policies and procedures of Community Catalysts of California.
  • Prepare and maintain current staffing schedules and coordinate new staff training schedules. 
  • Be part of the on-call rotation as needed and required.
  • Monitor and report to case responsible person on client and employee activities.
  • Complete CSPs, monthly/quarterly reports, evaluations, intakes and assessments under the direction of the case responsible person.
  • Review activity logs of clients and report significant events or concerns to case responsible person.
  • Provide direct service at least 60% of logged time to ensure clients service is rendering per their CSP.
  • Other duties as assigned.

WORK SCHEDULE:  The Direct Services Supervisor is expected to perform duties assigned during regular working hours, which is normally 40 hours per week.  However, please note that workweeks will be determined by the needs of the clients.  The Direct Service Coordinator may be called upon to work in the evenings or on weekends.  No holiday or overtime may be worked without the prior approval of the Regional Manager/Program Leader or his/her designee. 

Qualifications

SKILLS & EDUCATION REQUIREMENTS:

  • 18 years of age or older.
  • High School Diploma or Equivalent.
  • Bachelor’s degree or 3 years equivalent experience and/or educational equivalent in working with individuals with disabilities. 
  • Aptitude for confidentiality, organizational skills and good interpersonal skills.
  • Certification in an approved First Aid and CPR class; or successful completion within 45 days of the date of hire.
  • Insured, registered vehicle in safe working condition, and proof of current CA driver’s license.
  • Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.
  • A Direct Service Coordinator works as scheduled by the Regional Manager and/or his/her designee.   His/her schedule may be either part or full time, depending on the needs of the clients and Community Catalysts of California.  Overtime may be required as dictated by operational needs; however, no holiday or overtime may be worked without the prior approval of the Direct Service Coordinator's supervisor.
  • Successful completion of pre-employment drug screen and background check.

 

PHYSICAL REQUIREMENTS: 

  • Some of our clients are physically impaired and require assistance into and out of wheelchairs and other assistive devices.  The Direct Service Coordinator must be able to perform such tasks on an urgent or emergency basis.  Many of the jobs/activities in which our clients engage require physical exertion on the part of the Community Catalysts of California staff person (i.e., walking (75% of workday), climbing stairs (10% workday but on a frequent basis), lifting (10% of workday but on a frequent basis), stretching, bending, etc.).  A Direct Service Coordinator must have the ability to talk, hear, and see in order to assist the client in his/her quest for independence.
  • The Direct Service Coordinator is responsible for the safety and well-being of a disabled client they are providing service for. Must be able to react appropriately and quickly in the event of an emergency such as client elopement, a natural disaster, fire or other unforeseen circumstance. An appropriate reaction might be having to follow or, help the client evacuate in the event of a fire or emergency which may involve ascending or descending of stairs, walking a significant distance, or being able to summon emergency responders.
  • Appropriate and professional dress is expected.

  • In the community the employee will be working in community environments, employment sites and office environments. Potential exposure to noisy environments at job sites. Exposure to blood borne pathogens could occur. (Hepatitis B vaccines are offered to all employees at no cost to the employee.)
  • The Direct Service Coordinator must be capable of making on-site visits to clients. Often these on-site visits are at a distance from the office and no regular means of public transportation exists. Overnight travel is rare but may be required for training and as part of a participant's program. The Direct Service Coordinator is expected to perform duties assigned during hours needed to meet client needs, which means that the schedule may include evenings, weekends, holidays or other non-typical work hours.
