Minimum in-office: 2+ days/week at Milwaukie Medical, 1+ day/week at Admin (Tigard), and ability to travel to other medical clinics at least once a month.

SUMMARY
The Substance Use Disorder (SUD) Case Manager provides a full range of case management services for patients diagnosed with a Substance Use Disorder (SUD). This role is responsible for identifying, referring and authorizing appropriate services for patients assigned to them. Major responsibilities include assessment of client’s needs, determining level of care, client service plans, and facilitating the client’s treatment to address their problems relative to drug or alcohol usage when warranted. This employee will also perform the work in accordance with established regulations, policies, procedures, and supervisory review.
Essential Job Duties
- Conducts interviews utilizing the appropriate assessment practices to obtain the necessary information from patients and families (if applicable) for the identification of needed services.
- Treats patients in a manner informed by trauma informed care principles.
- Determines appropriate level of care based on placement guidelines.
- Determines appropriate funding arrangements and authorizes services for clients with drug or alcohol problems.
- Orients and guides patients through the program, policy, procedures, and treatment processes.
- Updates the patient’s service plan as needed.
- Coordinates and provides access to available alcohol and other drug or ancillary services, such as housing, childcare, medical or financial services.
- Prepares for, and facilitates service planning meetings, and provides follow-up after meetings.
- Monitors patient treatment progress and performs reviews of patient health status.
- Prepares written reports and maintains patient records.
- Manages patient caseloads and carries out work independently.
- Embodies utmost professionalism, confidentiality, and helpfulness while in this role.
- Responds to routine daily correspondence.
- Completes projects and special assignments in a timely manner, as directed.
- Follows all NHC policies, processes, and procedures.
- Performs other related duties as assigned and required.
