Comprehensive Life Resources is looking for a qualified Case Manager to join our Adult Outpatient team in downtown Tacoma Washington.

Function:

Provide case management services to adults with chronic mental illness.  Facilitate community adjustment of consumers at high risk of institutionalization.  Serves as a member of an organization that welcomes, engages and serves individuals with complex wellness needs.           

 

Responsibilities:

  • Provide case management services to adults with chronic mental illness. 
  • Facilitate community adjustment of consumers at high risk of institutionalization. 
  • Provide a comprehensive assessment of the consumer’s needs, treatment planning, and independent living skill training.
  • Provide liaison with other community resources to help the client access supportive services.
  • Provide training and consultation to family members, landlords, and other caretakers in providing support to consumers.
  • Maintain clinical records and provide data and reports, as may be required for program monitoring and evaluation, in accordance with policies and procedures.
  • Be available for consult and crisis outreach as appropriate or needed.
  • Perform health and safety checks of apartments as required. 
  • Be a part of a multi-disciplinary team to coordinate services. This includes attending regular team meetings, consultations, and training as needed or required.   
  • Ensure that ethical and professional standards of practice are maintained.
  • Participate in Adult Services meetings and activities as required.
  • In addition to the above, perform other duties as assigned.

Qualifications:

  • Bachelor’s degree in social or behavioral science   
  • Must have ability to obtain Agency Affiliation Credential from Department of Health 
  • Valid Washington State driver’s license; proof of insurance.
