Comprehensive Life Resources is looking for a qualified Homeless Outreach Worker to join our team in Downtown Tacoma.

Function: Provide outreach and case management services to homeless persons with chronic mental illness and substance abuse issues. Serves as a member of the organization that welcomes, engages, and serves individuals with complex wellness needs.

 

Responsibilities

  • Provide outreach and case management direct services to chronically mentally ill homeless persons. 
  • Assume responsibility for assigned consumers, ensuring follow-up on referrals for necessary services and facilitate continuity of care. 
  • Provide services designed to enhance home fullness and community survival skills for consumers. 
  • Participate in intake assessments and treatment planning for consumers. 
  • Provide an inventory of housing resources and working agreements with local housing providers for increased accessibility to housing resources. 
  • Provide education to facilities, professionals, and other caregivers. 
  • Ensure that ethical and professional standards of practice are maintained. 
  • Maintain clinical records and provide reports in accordance with policies and procedures. 
  • Participate in Adult Services meetings and activities as required.  Coordinate work with other staff. 
  • Demonstrate ongoing commitment to increasing competence and knowledge in the mental health field. 

 

Qualifications: 

  • One year relevant experience in mental health related field.
  • Experience in the provision of case management services that demonstrate the ability to work with the mentally ill homeless population.
  • WA State credential/license.
  • Valid driver’s license; proof of insurance
