Comprehensive Life Resources is looking for a qualified Homeless Outreach Worker to join our team in Downtown Tacoma.

Function: Provide outreach and case management services to homeless persons with chronic mental illness and substance abuse issues. Serves as a member of the organization that welcomes, engages, and serves individuals with complex wellness needs.
Responsibilities:
- Provide outreach and case management direct services to chronically mentally ill homeless persons.
- Assume responsibility for assigned consumers, ensuring follow-up on referrals for necessary services and facilitate continuity of care.
- Provide services designed to enhance home fullness and community survival skills for consumers.
- Participate in intake assessments and treatment planning for consumers.
- Provide an inventory of housing resources and working agreements with local housing providers for increased accessibility to housing resources.
- Provide education to facilities, professionals, and other caregivers.
- Ensure that ethical and professional standards of practice are maintained.
- Maintain clinical records and provide reports in accordance with policies and procedures.
- Participate in Adult Services meetings and activities as required. Coordinate work with other staff.
- Demonstrate ongoing commitment to increasing competence and knowledge in the mental health field.
Qualifications:
- One year relevant experience in mental health related field.
- Experience in the provision of case management services that demonstrate the ability to work with the mentally ill homeless population.
- WA State credential/license.
- Valid driver’s license; proof of insurance
