Plan of Care:

             ESSENTIAL DUTIES AND RESPONSIBILITIES 

Support client in achieving service goals and objectives that appear formally and informally on client’s Plan of Care by working with the client one-on-one or in a group. Address all goals and objectives identified or provide written documentation as to why a goal/objective is not being addressed.

Ensure health and safety for each client by following Easter Seals’ established policies and procedures.

Implement written activity plans provided; execute instructions with client input; prepare and maintain clients' schedules to ensure good organization while allowing flexibility.

Attend plan of care meetings and team meetings to discuss strengths, needs, and plan development as requested.

Share information on job needs, changes in client behavior, accidents, and emergencies with the Direct Care Supervisor and parent/guardian as necessary and appropriate and in a timely manner.

Respect and promote client rights, promote wellness, and adhere to client-centered approach.

Other duties as assigned.

Documentation:

Submit online clear and descriptive summaries that pertain to the client’s goals and that document time, date, and provision of service.

Prepare online time sheets.

Complete an expense report, documenting mileage driven while working on specified activities with the client in the community.

Complete required reports for unexplained changes in client behavior, accidents, and emergencies.

Submit all paperwork according to schedule.

Professional Development:

Attend all required training for the position, annual in-service, and twelve hours of continuing education annually.

Background/Drug Testing:

Obtain and maintain clearance through the Adult and Child Maltreatment Registry Checks

Obtain and maintain clearance through the Criminal Background Check

All employees must complete drug testing in accordance with Easter Seals’ Staff Handbook.

Transportation:

Provide transportation in the client's community allowing clients to accomplish objectives in their Plan of Care such as access to the community, social functions, shopping, and work as needed.

Have the ability to access emergency service systems.

When necessary, supervise client training regarding proper use of public transportation. Assist in securing appropriate bus cards, schedules, etc. Continue to supervise clients' use of public transportation until the client can travel independently.

Demonstrate safety procedures at all times in vehicle by using seat belts, not using cell phone or engaging in any other prohibited conduct.

Remain in compliance with Easter Seals driver safety policies and procedures as outlined in Staff Handbook.

Qualifications

Have a high school diploma OR have successfully completed a GED or have a minimum of two years verifiable successful experience with individuals with developmental disabilities that may be used in lieu of the aforementioned qualifications.

Certificates, Licenses, Registrations:

Arkansas Drivers License

Own transportation with current valid insurance

Maintain a valid CPR/First Aid certification.

Clerical/Computer Skills:

Knowledge of basic administrative and clerical functions and systems such as word processing, managing files and records, use of electronic health records, and other office procedures and terminology.

Technology:

Must have a Smart Phone or other electronic device with the capability to connect to the internet to complete timesheets, progress notes and to send and receive text messages as required.

Language Skills:

Have the ability to communicate effectively with clients and families.

Ability to read and interpret job-related documents, procedure manuals, and Plan of Care.

Ability to write routine reports and correspondence.
