Title:                           Outreach Specialist

Reports to:                 Transitional Specialist Coordinator

Status:                        Full Time, Regular, Non-Exempt

Location:                    Based in Harlem, with travel throughout New York City

 

Position Summary:

Responsible for engaging clients and supporting them as they transition back into the community to ensure connection to primary medical care and Medical Assisted Treatment (MAT).

 

Essential Duties and Responsibilities: 

  • Conduct client follow up and monitor client progress
  • Conduct outreach for clients who are not engaged in services; via telephone, field visit, sending correspondence
  • Assist Transitional Specialist with escorting clients via agency van as needed
  • Enter progress notes in CLIMB, Fortune Society’s proprietary client database
  • Conduct program-specific outreach activities to external service providers
Qualifications

 

  • High school diploma or GED/HSE preferred;
  • Experience working as an Outreach Worker a plus but not required;
  • Ability to travel throughout the 5 boroughs to escort clients to appointments;
  • Computer data entry skills;
  • Experience/Knowledge in MAT a plus but not required;
  • Good communication skills, bi-lingual English/Spanish preferred; and
  • Ability to work under strict confidentiality and openness to guidance and supervision.

Travel Requirements:

Ability to travel throughout the 5 boroughs by public transportation and via van.
