The facilitator consults with the DCFS/POS case worker and the child and family team to develop and implement a family centered, strengths based, trauma informed plan that addresses the needs of the child and family while using their natural supports and strengths to reach their permanency goals. The current openings will be serving families in the Effingham/Olney area.

Job duties include:

• Consult with the caseworker and child and family team as identified by the family
• Assist families and their team in identifying, recruiting and engaging natural supports
• Attend child and family team meetings for updates and further planning
• Assist teams in identifying strengths and needs and establishing goals
• Provide leadership in the development, implementation and evaluation of individualized plans using the phases and activities of Wraparound
• Define a safety plan to address behavioral escalations if needed
• Assist the case worker in preparing a meeting outline/agenda with the family; goals, child/family strengths, concerns, barriers to reaching goals, etc.
• Identify barriers to the process and develop strategies to overcome them
• Ensure the Wraparound process is done with fidelity to the model whenever possible
• Ensure all required confidentiality and other compliance documents are current
• Educate families, providers and the community regarding the Wraparound Process
• Complete all required documentation
• Other duties as assigned

Minimum Qualifications: Bachelor's degree in Social Work or other Human Services field.

Desired Qualifications: Previous case management experience in social services and/or child welfare.

Must be able to pass pre-employment drug screen, background check and have a clean driving history.

Must be able to work flexible hours to meet the needs of families, including some evenings and occasional weekends.

Required to work in clients' homes and community settings.

Required to travel to sites throughout the identified service area.
