SUMMARY: In accordance with RNP's Mission Statement, Philosophy of Client Care and Code of Ethics as stated in the Personnel Policies and under the supervision of the Department Director provide general direction and supervision to departmental staff and manage an assigned caseload of clients. Provide Rapid Rehousing case management to clients impacted by gun and gang violence and the criminal justice system who are literally homeless or experiencing housing instability and may have challenges with substance abuse and co-occurring mental health issues by performing the following duties.

HOURS: Tuesday - Friday 9:00 am - 5:00 pm; Saturday 4:00 pm - 12:00 am

Specific Duties and Responsibilities: All duties are subject to accommodation in accordance with the American Disabilities Act. (ADA)

  • Provide Rapid Rehousing case management for up to 15 households.
  • Assess the individual’s safety, finances, employment and level of functioning.
  • Assertively links individuals to community resources and other natural supports.
  • Establish partnerships with landlords.
  • Serve as advocate for clients in obtaining necessary services.
  • Adhere to all standards as it relates to documentation requirements.
  • Develops and documents person centered housing plans and update monthly.
  • Conduct home visits and on-site individual sessions.
  • Conduct unit inspections.
  • Provide support services for maintaining housing stability.
  • Works in partnership with Judicial Branch, Court Support Services Division (CSSD) Department of Housing (DOH) and ACT.
  • Participate and prepare for supervision weekly.
  • Attend and prepare for all Rapid Re-housing and PHS staff meetings.
  • Prepare material to conduct group facilitation.
  • Maintain organized files and adhere to timely data entry into HMIS and other data systems.
  • Assumes all other appropriate duties as deemed necessary by the supervisor.

Qualifications

CORE COMPETENCIES

Bachelor's degree in the behavioral health field and two (2) years experience in counseling, case management or rehabilitation services preferred, or equivalent combination of education and experience.  Co-occurring competencies and/or credentials are a plus. Two years supervisory experience preferred.

  • Knowledge of principles and techniques of counseling, crisis intervention, and/or case management. 
  • Understanding of and ability to provide integrated care for individuals with co-occurring disorders.
  • Ability to gather, analyze and evaluate information pertinent to the care of persons served. 
  • Knowledge of the physical, psychological, social and economic effects of substance abuse, homelessness, and mental illness. 
  • Knowledge of community resources. 
  • Ability to develop relationships and maintain a resource listing of Landlords, Property Management companies, and Realtors to assure Rapid housing solutions for participants.
  • Considerable knowledge of federal and state laws and regulations regarding client confidentiality. 
  • Ability to establish and maintain cooperative professional relationships. 
  • Excellent communication skills. 
  • Ability to project a professional images

 

 

CERTIFICATES, LICENSES

Incumbents may be required to secure professional certification and/or license.  A valid Connecticut driver's license may be required.
