Essential Duties & Responsibilities:


  • Provide case management linkage services to clients admitted to The Family House residential treatment program.
  • Work with clients individually and set up appointments in the community and on-site that foster self-sufficiency.
  • Perform orientation to help clients get acclimated to residential treatment.
  • Teach applicable psychoeducational classes to clients.
  • Perform discharge planning by meeting with clients on a weekly basis or as needed during their 90 days in treatment to assist arranging post treatment housing, linkages with medical and behavioral healthcare providers, employment services, peer recovery support services, and in addressing barriers such as lack of ID documents, and bus passes for transportation and cell phone or computer for attendance at IOP and online meetings.
  • Ensure access to appropriate pediatric medical care, including well-child visits and developmental screenings.
  • Act as a liaison with the clinical team to provide information to maximize client care.
  • Develop and maintain a working relationship with community agencies that provide services to clients while in treatment and post discharge.
  • Ensure access to appropriate primary medical care, including prenatal and postpartum care and reproductive health services.
  • Assist eligible Clients with obtaining community assistance/support prior to discharge.
  • Ensure that any client being discharged, or unsuccessfully discharged due to non-attendance or non-compliance, is referred to the Linkage Specialist for follow up support post-discharge.
  • Facilitate enrollment with IOP admissions team or other step down, as applicable.
  • Perform charting function; creating, printing and filing all necessary documentation, to include but not limited to progress notes (individual, group, educational and administrative).
  • Work with Director of Recovery and Alumni Services on quarterly data reporting.
  • Coordinate with Child Protective Services, and/or court system, and other child placement situations, to increase access to child(ren) not residing with pregnant woman and/or woman with Dependent Children.
  • Other duties as assigned

•     Experience working with high-need, low-income families

•     Working knowledge of substance-abuse and mental-health disorders

•     Effective written and oral communication skills

•     Ability to establish effective rapport with clients

•     Ability to work flexible hours

•     Must be able to use basic computer software

•     Must be CPR/First Aid certified upon hire or within 90 days of hire

•     A valid Texas driver’s license and a clean driving record



•     Bilingual Spanish-English


Education and Experience:

•     Bachelor’s Degree, preferably in a related field

•     LCDC license or CI designation, OR 2 years minimum experience in a case management role

•     Experience working in the Substance Use Disorder field


Supervisory Responsibilities:

•     This job has no supervisory responsibilities


Supervised By:

•     Director of Recovery and Alumni Services

Physical Demands/Work Environment:

•     Communicates orally with management, co-workers and the public in face-to-face, one to one, and group settings.

•     Regularly uses a telephone for communication and office equipment such as computers, copiers, scanners and printers
