SUMMARY:
In accordance with RNP's Mission Statement, Philosophy of Client Care and Code of Ethics as stated in the Personnel Policies and under the supervision of the Coordinator the Case Manager will assist in the provision of quality care for persons served by performing the following duties:
SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).
- Knowledge of best practices in providing services for individuals with mental health and substance use disorders such as client centered care, motivational interview and trauma informed care.
- Maintain boundaries and ethical standards understand and demonstrate an ability to maintain appropriate professional boundaries when working with client and uphold ethical standards, e.g, no dual relationships, minimal disclosure, etc.
- Develop and document client centered care plans, record and monitor clients progress regarding HIV/AIDS, finances, employment, food, basic needs, housing, physical health, mental health, and substance abuse related issues.
- Link clients to community resources and other organizations and arrange appropriate referrals and placements for program participants. Work in collaboration with service providers.
- Maintain a schedule of all clients 1:1 sessions/ appointments. Provide each client with a consistent scheduled time for session.
- Facilitate weekly Life Skill groups and provide information on topics such as communication, focus, self-control, problem solving, decision making, self- awareness, etc.
- Acknowledge and document all behaviors of clients that are considered inappropriate or cause concern. Attempt to redirect clients in a respectful manner as needed and offer support as deemed necessary.
- Monitoring and conducting hourly rounds throughout the shift and recording all pertinent observations and details in the Logbook.
- Adhere to all standards as it relates to documentation requirements.
- Conduct 1:1 individual session with clients weekly and submit monthly contact forms to Ryan White Services.
- Perform medication monitoring and front desk coverage.
- Attend and participate in weekly staff meetings and quarterly all staff meetings.
- Supervision come prepared monthly with an agenda when seeking supervision.
- Assume all other appropriate duties as deemed necessary by the supervisor.

- Excellent Communication Skills
- Ability to work under minimum supervision and to assume initiative.
- Ability to establish projects and maintain cooperative professional relationships.
EDUCATION AND/OR EXPERIENCE:
- Bachelor’s degree in behavioral science and two (2) years’ experience in case management preferred, or equivalent combination of education and experience.
- Incumbents may be required to secure professional certification and/or license.
- Valid Connecticut Driver’s license
