SUMMARY
In accordance with RNP's Mission Statement, Philosophy of Client Care and Code of Ethics as stated in the Personnel Policies and under the supervision of the Clinical Director, assist in the provision of quality care for persons served by performing the following duties.

CORE FUNCTIONS:
The Case Manager - After Care is responsible for planning and organizing client aftercare program referrals, including arranging transportation plans for clients scheduled to complete treatment. The Case Manager - After Care must have the skills to develop and maintain professional networking abilities in collaboration with all internal and external client transportation and referral programs. The Case Manager - After Care fosters positive relationships between RNP and referral programs while coordinating a continuum of care and maintaining a positive professional image as an RNP representative.
- Serve as a client advocate, facilitate and follow up on all client screenings, and ensure all requested client documentation is expedited with prompt completion and the highest priority.
- Have a good understanding of HIPAA, ROIs, and coordinating with client conservators.
- Understand ASAM levels of care for clients completing a 3.7-level program.
- Assist clients with defining specific goals and objectives for appropriate levels of aftercare.
- Collaborate with clients and a multidisciplinary clinical team, working closely with clinicians, admissions, and nursing staff to communicate client discharge progress or barriers.
- Following up with client discharge medications by collaborating with the PA and nurses to ensure the client’s discharge medications are ready for the client’s discharge, including connecting clients with their continued MAT services.
- Maintain the program’s daily bed roster and update the DMHAS daily bed census.
- Ensure clients obtain the necessary clinical wrap-around services in the community and help clients fully understand their transition plans to ensure a clear understanding of their plans following inpatient treatment.
- Understand and use Electronic Health Records (EHR) and the skills to complete discharge documentation that reflects the client’s collaboration, and the understanding of their transition discharge plans meeting state and federal auditing standards.
- Ensure clients receive their property, physical medications, and discharge certificates.
- Facilitate Aftercare Informational groups and psycho-educational groups as needed.
- Assist with observing meals, laundry, and smoke breaks.
- Assist with the intake of new clients.
- Assists with the coordination of daily discharges and discharge transportation
- Assumes all other duties as deemed necessary by the supervisor.
